delivering naturally with a breech baby

Okay so my situation is a bit crazy, but i'll try to explain a little but.. But right now my baby is breech, I'm only 29 weeks, but I have a bi-cornet uterus. So really the horn that she is in is only half the size of the normal uterus. My doctors dont that she will flip around this late on and are already ready to set a c/s date up... I've delivered 3 babies naturally and want her to be natural also (one of with was a stillborn baby, very hard to do, and was adviced not to, but I didnt want it any other way).. All head done by 20 weeks and never flipped around..

My theory is my uterus may be smaller but my cervix and who-ha isn't so why shouldn't it be that same as a regular breech child birth..Also my doctors worry because I had a cerclage with my last pregnancy and there was alot of scarring and it was hard for me to dialate with my daughter, I have no cerclage now and have had a birth since then..

I guess I was wanting some information on a natural breech child birth, and maybe some support since no one seems to think its a very good idea..

(Now let me add, that if my baby was in ANY kind of danger and needed to have a c/s I would be the first one to say lets do it.. I just dont see why a breech birth is even being ruled out so fast?)

Oh and to make my situation even harder, my ob told me last week that he was quiting his ob practice effective this past week, so he wouldn't be the one to deliver, the other ob in his clinic would be, but it would be ideal if I just opted to have to c/s because he could just sign on as a regular doctor and be there..  I'm so confused on what to do right now.. I see a perinatologist also do to high risk and she said no to a breech delievery also but she dont even have rights to hospital I'm delivering at.. I travel over 100 miles every other week to see her.. and now my ob quit and there is the other doctor in his clinc, and he says a c/s would be easier so he knows when and where to be at what time.. (No complications.. is what I think he said actually)
(There is 2 other doctors in our town, but I wont see them because that was the clinic I went to when we lost our son and they were .. well lets just say **** words I dont like to say anymore)

LOL, your who-ha.............Anyway, I know delivering breech is possible but I THINK it depends on what type of breech (Frank, footling, etc). I would just do a google on that to see for sure. Also, check out spinningbabies.com. It 's a website devoted to getting babies in the optimal birthing position. I'm not real familiar with the website but lots of natural birthers have suggested it for women with breech babies. Sorry I couldn't tell you more.

    Congratulations!!

Perhaps you could try ican-online.org and see if there is a local chapter. Eventhough you haven't had a c-section, they may be able to point you in the right direction of a doc who'll consider a breech delivery.  Have they even considered flipping her?  My last OB, in Atlanta, said that he'd do a version early on...20-ish weeks, if baby was breech because he said that most of the time, the baby will stay and not flip around. He said that he'd continue to flip the baby head down, but he also did breech deliveries...if he felt it was safe.

Re: delivering naturally with a breech baby

it's totally possible but it depends on the position baby is in- and most ob's will tell you what positions they will and wont attempt and can explain why- but as far as the scarring interfeing with dilating- i wouldn't schedule a c-sec based on that- i'd say if everything else is go attempt to labor on your own and dilate and then if there's a problem you can go for the c-section... I would also push to have them attempt to turn the baby- because regardless of whether your uterus is smaller than normal- they can still use the same techniques to try and flip the baby... good luck and let us know!! :)

oh- and i totally wouldnt let him schedule the c-section because of him wanting a more convenient delivery time... that's just crap.. sorry :P i'd have some words for him for even thinking that would fly... doesnt sound like he cares much either way- and i'd just get myself aquainted with the other dr and make it work.. personally.. they dont have the right to force you into a c-section just because it's breech- but i'd talk to the perinatoligist and discuss my concerns about it- maybe ask her why- and say well as long as baby's in such and such a position i'm going to try for it anyway.. because honestly- you're still the boss.. there are some really good videos online of breech births btw if you google em :)

Re: delivering naturally with a breech baby

Your situation is tricky. I'd definitely seek help from a midwife...even if just for prenatal care. A physician sees breech and instantly thinks "section." A midwife sees breech and thinks "how can we make this baby turn?" So, I'd keep your OB, but get some opinions from a midwife, practicing midwife or nurse midwife.

Cervical scarring can impede dilation, but not always. I wouldn't schedule a CS based on that, like the other poster said.

Bottom line is that I wouldn't schedule a section now, regardless. The fact is that you don't know what can happen from now until then. And sections can be done at anytime of the day. People schedule sections days in advance all the time. Don't put yourself through the stress of a deadline like that.

BTW: What makes you high risk?

Thank you for your input.. I see my peri. again tomorrow and will discuss things over again differently this time, its the first time ive seen her since finding out my ob quit.

What makes me high risk, I guess alot of things now.. I had my first at 33 weeks(water broke), 2nd was born still at 20 weeks (water broke, had started dialating at 14 weeks), then a miscarriage, then #4 (was twins but baby #2 had stopped developing and disappered by 14 weeks, started dialating at around 12 weeks and had a cerclage placed.. Found out I only have 1 kidney and had to have a stint placed because the pregnancy was causing a tube to close up and make my kidney and bladder spasm. Plus I have a bi-cornet uterus..

    So confusing..

There is an accupuncture technique called moxibustion that has great results at turning babies. Bout 85%. The only breech that cannot be delivered vaginally is transverse or sideways. Otherwise you just have to find a provider that is willing and trained to deliver breech babies vaginally. Problem is since standard practice is to automatically section for breech not many drs are trained to do it. If you don't want a scetion your best bet is to find a midwife. Good luck!

What city are you in?  I'll send an email out to all the ICAN girls if you don't want to join there yahoo group.  You can also try seeing a chiro that specializes in the webster technique.  Also check out spinning babies.com  I would defenitally find a new doc.  I'll give you some more input tomorrow, bed is calling.

I'm also part of a breech google group & they have been creating a list of docs that will perform breech deliveries across the country.

I can sympathize.  I was forced into a c/s for a breech baby myself.

Re: delivering naturally with a breech baby

I have some information on breech options...here's a few websites I had bookmarked (I'm a doula, I keep these things handy)

    http://www.birthinternational.com/articles/andrea13.html

    http://www.brooksidepress.org/Products/Military_OBGYN/Textbook/AbnormalLandD/Breech.htm

    http://www.aims.org.uk/Journal/Vol10No3/breechCSvsNormal.htm

    http://emedicine.medscape.com/article/262159-overview

What it boils down to is not a provider that will DO a breech delivery, but one that has been trained and has experience doing breech deliveries.  Many will only do Frank or complete breech presentations...not a footling or kneeling.  It helps that you have had a vaginal delivery before, that should make things easier, some providers will only allow a breech delivery  on moms who have had a vaginal delivery before.

Talk to your care provider, talk to doulas in the area as well, they may know of providers that will let you attempt a breech vaginal delivery.

    Good luck!!

Re: delivering naturally with a breech baby

Hey everyone.. Thank you so much for the information so far.. I'd love more help if I can get it..

I'm 32 and 6 days now and still breech.. I have been seeing a chiropractor for 3 weeks now using the webster technique and will keep going until..

I live in Mo and looked all over and there are NO midwives any near me that are birthing anymore.. I am SO againest a c/s but thats all they (my doctors and now my "new" c/s doctor) are talking about.. I dont go back until the 5th, my original c/s date, but I told them I wouldn't come unless they pushed it back at least two more weeks.. But hopefully by then I can get more information gathered on someone that would be willing to preform a breech delivery in my area or the baby flips around..

    FLIP BABY FLIP!

Re: delivering naturally with a breech baby

My daughter was also a "footling" breech. have you tried the positions recommended at http://www.spinningbabies.com/techniques/the-inversion? There is even a short video to show you how to do it. Sometimes it can help babies turn. They also recommend putting very cold things on your belly on/close to the baby's head or butt. babies don't like it so it encourages them to move. There is also the option of external cephalic version (ecv) usually done around 37 weeks where the practitioner uses an u/s and monitors the baby's HB while  manually manipulating the baby to turn. it has slight risks of causing abruption, ruptured membranes and labor and if your uterus is very tight can be hard to do (in this case they sometimes give drugs to relax your uterus before they do it) it takes a trained practitioner to do (and again these are hard to find cause they usually jump straight for a c/s for breech :( it does carry some risks although they are smaller than the risks assoc w/c/s.

Finally, although it is a very extreme option--you have the right to refuse a c/s regardless of the reason it is recommended. If you refuse the c/s they will have no choice but to deliver your breech baby vaginally. The EMTALA (Emergency Medical Treatment and Labor Act) says that once you are within 250 feet of a hospital they cannot turn you away until the baby and placenta are delivered and both mom & baby are stable. Federal and state laws, as well as the AMA, hospital licensing regulations and court precedence all support your right to refuse any treatment regardless of the outcome. In the past they would get a court order to do a c/s anyway. In each case the courts (even the Supreme Court) has held that this is unacceptable, constitutes battery and violates your rights to informed consent/refusal.Your right to consent/refusal is absolute! I would like to add/caution that if there is no one that is trained to deliver a breech baby it can be dangerous. And although it is w/in your rights to do so refusing a c/s at that point could carry serious risks.

    FLIP BABY FLIP!!!!!

I know someone who has that kind of uterus as well. She had three breech babies- the first two were cesareans, & the 3rd was born breech at home complication-free :) Email me at LNCooper83@yahoo.com & I can pass along her email address to you. She's got TONS of info on the subject! :)
